Mine is under the floor and no issues. It's good copper multistrand, the only downside in some situations is it's stiffness which can put pressure on terminals if you don't take care to shape it a bit.
Warming it with a hairdryer can help you shape it more easily.

Are you talking about flame tests? With the low voltages and currents fire is not a problem with speaker cables. I can't see it setting fire to anything even if it's insulation is compromised, shorting out might screw your amp though.

Naca 5 is not approved for in-wall installation, it's not even UL or CSA approved (It should be marked on the cable itself). There's no problem with the regulations when it's outside walls, as it's this way considered by the regulations like a piece of furniture, because it's not carrying mains AC but an audio signal.
However, there's a solution for in-wall installation, use a CSA approved (Canada) fire proof or flame retardant (minimum is FT-4) sleeve or duct (available at any good specialized electrical/electronic components store), so you'll be OK with the electrical safety regulations.
